2021-2022 Free and Reduced Application Packet
The 2021-2022 Free and Reduced Lunch Forms are available. Your 2020-2021 eligibility is good until September 30th, 2021, but a new form must be completed by October 1st, 2021.

YOU MUST DOWNLOAD THE APPLICATION FORM BEFORE COMPLETING AND SENDING IT OR YOUR RESPONSES WILL NOT BE SAVED.

To file a program discrimination complaint, complete the USDA Program Discrimination Complaint Form, AD-3027, found online at How to File a Program Discrimination Complaint and at any USDA office or write a letter addressed to USDA and provide in the letter all of the information requested in the form.
